nghttpx: Use NGHTTP2_NV_FLAG_NO_COPY_NAME and NGHTTP2_NV_FLAG_NO_COPY_VALUE

For both HTTP/2 frontend and backend.

Also adds http2::stringify_status to optimize status code
serialization.
